Band Fees 2018/2019

This section includes the Band Registration, Parent Volunteer, and Medical form. These required forms are to be filled out for each student before July 16th, 2018.

All forms are to be completed / submitted AND fees are required to be paid in full (unless other arrangements have been made with Bob Myers, Band Director) before Band Camp, July 16th, 2018. YOUR STUDENT WILL NOT BE ALLOWED TO PARTICIPATE IF THE MEDICAL FORM IS NOT ON FILE July 16th!

2018/2019 BAND REGISTRATION AND FEES

Filling Out This Form Guarantees you understand the Following:

(1) There is a spot reserved specifically for the student named below in the marching drill.
(2) We understand all attendance guidelines and that it is mandatory that each participant attend the entire band camp.
(3) We understand all behavior guidelines set by EACS and the Band director.
(4) We are aware that band fees are non-refundable fees and must accompany a signed and completed student medical form by February 23rd, 2018 in order to participate.
(5) We understand Uniform Care and Instrument Care.
(6) We understand that there is a student volunteer minimum requirement of 3 events in the fall semester and 3 events in the spring semester.

The "Roar of the Lions" Band is an active performing ensemble composed of those students enrolled in the Concert Band, Pep Band, and Marching Band. In addition to the continued study of advanced musicianship and color guard techniques, this band is an active ensemble with many performing opportunities utilizing these acquired skills.

By filling out this agreement, you understand and agree to pay these band fees. You understand and agree that all fees are NON-REFUNDABLE and give your permission for your student to participate in the Leo Band Program for 2018/2019.

NOTE: We use email as the primary source of communication to parents regarding schedule information and events. If you prefer not to receive emails, then you will not be sent any information about practice scheduling, events, etc...

Parent Volunteer Opportunities - Parent involvement is critical to the success of our band program. Please select at least one area below that you would like to help with. If you cannot volunteer full time for a certain area, that's ok. You don't have to be available for all event dates, so just indicate your interests and we will work you into the schedule. NOTE: All parents are expected to volunteer.

Medical History

NOTE: No student should have any medication not prescribed by a physician as according to school policy. If medication is needed while traveling, it must be accompanied by a note from the parent requesting that it be taken during this time. Also, medication must be in the original prescription bottle. The nurse or chaperone shall dispense with these medications as designated by the Director. It is the responsibility of the parent and/or student to advise of any changes.

Insurance

IF YOU OR THE EMERGENCY CONTACT CANNOT BE LOCATED IN CASE OF A SEVERE EMERGENCY, YOUR STUDENT WILL BE TRANSPORTED BY AMBULANCE TO THE NEAREST HOSPITAL.
